The ruler shows a standard 1 mL U-100 insulin syringe, where 100 units equal 1 mL. Units mark the same volume on every syringe size, so a draw of 8 units is identical on a 0.3 mL, 0.5 mL, or 1 mL syringe.

How the math works
Dividing the powder in the vial by the volume of bacteriostatic water you add gives the concentration. A 5 mg vial reconstituted with 2 mL of BAC water holds 2.5 mg per mL. Dividing your target dose by that concentration gives the volume to draw, and multiplying the volume by 100 converts it to units on a U-100 insulin syringe. A 250 mcg dose at 2.5 mg per mL works out to 0.1 mL, which reads as 10 units.
Adding more BAC water never changes how much peptide the vial holds. More water dilutes the mix, so each dose becomes a larger and easier-to-measure draw, while less water concentrates it and makes each draw smaller.
